I am not a pathologist, nor am I a physician, but I am an advocate for accurate information (especially for men trying to make a cancer treatment decision), and a 13 year advocate for my husband’s advanced prostate cancer, among other things. A description of our method and research can be found in the paper: Automated deep learning system for Gleason grading of 2021-01-01 To determine the Gleason score, the pathologist uses a microscope to look at the patterns of cells in the prostate tissue. The most common cell pattern is given a grade of 1 (most like normal cells) to 5 (most abnormal).

2021-04-02 · Gleason Grading System Grade group 1: Gleason score 6 or lower (low-grade cancer) Grade group 2: Gleason score 3 + 4 = 7 (medium-grade cancer) Grade group 3: Gleason score 4 + 3 = 7 (medium-grade cancer) Grade group 4: Gleason score 8 (high-grade cancer) Grade group 5: Gleason score 9 to 10 After adjusting for Gleason score, the rates before and after 15 years did not differ (rate ratio 1.1, CI 0.6 to 1.9). Men with low grade tumours had low risk of dying from prostate cancer over the next 20 years; the mortality rate increased with increasing Gleason score (table). The Gleason scoring system has been revised recently. Only three Gleason grades, 3–5, are currently recognized in the revised system instead of the five original Gleason grades, 1–5. Original Gleason grades 1 and 2 are now included within the Gleason grade 3 tumors and considered to be nonaggressive. Gleason grade patterns are assigned from 1 to 5: 1: well formed glands (least aggressive) 5: lacking gland formation (most aggressive) Most tumors demonstrate two histologic patterns and this is expressed as a Gleason score.

The Gleason Score ranges from 1-5 and describes how much the cancer from a biopsy looks like healthy tissue (lower score) or abnormal tissue (higher score). In 1966, Dr. Donald Gleason (Cancer Chemother Rep 1966;50:125) devised grades of 1 - 5, based on glandular architecture and microscopic appearance using a 4X - 10X objective eyepiece, that were shown to predict outcome in prostate cancer A Gleason score of 7 can be derived from differing primary and secondary grades, for example 3 and 4, or 4 and 3. This can be significant because a primary grade of 3 indicates that the predominant Most tumors demonstrate two histologic patterns and this is expressed as a Gleason score. The two most common grade patterns seen in a tumor are expressed together and summed. Gleason scores can be expressed as their individual components or as the total Gleason score, eg 3+4=7.
Cancers with a higher Gleason score are more aggressive and have a worse prognosis. Pathological scores range from 2 to 10, with higher numbe Gleason score 7 (or 3 + 4 = 7) Grade Group 2: Most cells still look similar to normal prostate cells. The cancer is likely to grow slowly: Gleason score 7 (or 4 + 3 = 7) Grade Group 3: The cells look less like normal prostate cells. The cancer is likely to grow at a moderate rate: Gleason score 8 (or 4 + 4 = 8) Grade Group 4: Some cells look abnormal. The cancer might grow quickly or at a moderate rate A Gleason score of 6 is low grade, 7 is intermediate grade, and a score of 8 to 10 is high grade cancer. It’s also important to know whether any cells rated at Gleason grade 5 are present, even in just a small amount, and most pathologists will report this.
